CRITERIA FOR SERVICES
The people who live at Beverly Farm have been diagnosed with an intellectual disability or related condition.
INTELLECTUAL DISABILITY
A person diagnosed as intellectually disabled exhibits significantly sub-average intellectual functioning concurrently with deficits in adaptive behavior manifested before age 22. Significantly sub-average is defined as having an IQ of 70 or below, based on standardized measures of intelligence.
RELATED CONDITIONS
A person with a related condition lives with a severe, chronic disability that meets all of the following criteria:
- It is attributable to Cerebral Palsy or Epilepsy or any other condition other than mental illness, found to be closely related to intellectual disability because this condition results in impairment of general intellectual functioning or adaptive behavior similar to that of a person with intellectual disability and requires treatment or services similar to those required by a person with intellectual disability.
- It is manifested before the person reaches the age of 22
- It is likely to continue indefinitely
- It results in substantial functioning limitations in three or more of the following areas of major life activity:
- Self-care
- Mobility
- Understanding and use of language
- Self-direction
- Learning
- Capacity for independent living

Who does Beverly Farm serve?
Beverly Farm supports adults with intellectual and developmental disabilities who need residential services, health and wellness support, and structured programs. We serve individuals with a wide range of abilities and support needs.
What types of residential options are available?
We offer multiple living options, including Intermediate Care Facility (ICF) cottages and group homes, Community Integrated Living Arrangements (CILAs), and private-pay apartments. Placement is based on individual needs, preferences, and availability.
How do I start the admissions process?
You can start by submitting an admissions application packet request form or by contacting our Admissions team by email or phone. We’ll begin with an initial conversation to learn about your loved one, answer questions, and explain next steps.
What information is required to apply?
We typically request recent medical and psychological evaluations, social history, guardianship documentation (if applicable), financial/funding information, and current support plans. Our team will provide a checklist and help you gather what is needed.
Can families visit before making a decision?
Yes. We encourage prospective residents and families to visit campus, tour living areas and program spaces, and meet members of our team so you can make an informed and comfortable decision.
How is placement determined?
Placement decisions are based on clinical and support needs, compatibility with available settings, safety considerations, and the individual’s goals and preferences. Our team works closely with each applicant to ensure a good fit.
What services are available on campus?
Services may include residential support, nursing supports, behavioral supports, therapy supports, recreation and enrichment programs, day services, employment programs, and individualized wellness supports.
What health and wellness supports are available on site?
Beverly Farm provides a range of on-site health and wellness supports, including nursing, medication support, behavioral supports, and occupational and physical therapy access.
How long does the admissions process take?
Timelines vary depending on availability of records, funding approvals, assessments, and openings. Our admissions team will keep you informed throughout the process and work to move forward as efficiently as possible.
Is there a waiting list?
At times there may be a waiting list for certain residential settings. Availability depends on the level of support needed and current openings.
How is Beverly Farm funded?
Funding may come through state programs, waiver services, private pay arrangements, or a combination of sources. Our team can help families understand funding pathways and requirements.
Can residents have visitors and home visits?
Yes. Family involvement is encouraged. Visiting, outings, and home visits are coordinated with the care team to support safety and individual preferences.
How do you communicate with families?
We maintain regular communication through care planning meetings, progress updates, and direct contact with staff and leadership. Families are valued partners in support and planning.
