admissions : payment considerations

If desired, we can assist you in acquiring Medicaid re-imbursement from the State of Illinois, Department of Human Services. The following eligibility criteria is necessary for Medicaid re-imbursement:

1. A Pre-admission screening (PAS) Agent must determine that the individual seeking services has a developmental disability and needs the level of care we provide. The State contracts with local service coordination agencies across Illinois to conduct Pre-Admission Screenings. Find your local PAS agency.
2. The individual needing services must meet the state’s income/assets guidelines for eligibility. Some sources of income like SSI or survivor benefits would be applied first to his/her cost of care and the state may pick up the additional cost.
3. If you are considering Beverly Farm as the new residence for a family member who currently lives out of state you may be eligible for reimbursement if the other two criteria above are met at time of admission. Beverly Farm staff will work with you throughout the process.
